  • the complete address, telephone number and fax number of your school or business
  • the shipping address and contact person
  • the quantity and product name or sku number
  • the shipping charges if applicable (for orders less than 25)
  • the accounts payable address and fax number
  • the purchase order number
  • the certificate of tax exemption if applicable
